This also applies to all patients who wish a partner or family member to speak on their behalf. By completing this request it will make things much easier when we are making contact with you. This consent will be recorded on your medical record.
In order to keep your medical records up to date we regularly run audits on the contact details we hold for our patients. Once a patient turns age 16 or does not have consent in place any duplicate telephone numbers (For example. same as a parent) will be removed from their record.
