Requesting access to historic medical records

The NHS app may allow you to view parts of your GP health record. If you would like to request access to historic information in your medical record, this requires a separate application.

To request historic online access, you will need to visit the surgery in person:

  1. Complete our Application for Historic Records Access form, available from our leaflet display in the waiting area or from the Reception desk.
  2. Provide two forms of identification so that our team can verify your identity. This must include:
  • One form of photographic identification
  • One proof of address

Once your application has been received, our team will review the request. Historic record access may require a clinician to review the records before access is granted. This is to ensure that the information is appropriate to make available online and that your privacy and safety are protected.

Please speak to Reception if you require an Application for Historic Records Access form or have any questions about requesting access to your historic medical records.

Family and carer access (proxy access)

Proxy Access allows you to use your own NHS App to access certain NHS services on behalf of another person, such as a child, family member or someone you care for.

How to request Proxy Access- You and your Proxy user must visit the surgery in person to set up.

You will need to:

  1. Complete our Request for Proxy Access form, available in our leaflet display in the waiting room or asking at our Reception desk in the surgery.
  2. Provide two forms of identification so our team can verify your identity. This must include one photographic ID and one proof of address, valid and in date or within the last 3 months

Our team will review the request, verify the ID and process your request. In some cases this does need to be reviewed by a clinician before access is granted.

Please do not use another person's NHS login details. Proxy access must be set up through the surgery to ensure patient information remains secure.
