How does it work?
When you visit your Castlegate & Derwent Surgery for care or treatment, you will be given the opportunity to give your feedback by answering a simple question about your experience:
“Overall, how was your experience of our service?”
You can rank your answer from "very good" to "very poor"
You will also be invited to add a comment. Your response is anonymous.
We have different options for you to complete the Friends and Family feedback:-
- Complete a form in reception and post the postcard in a ballot box in Reception
- Click on the button at the bottom of this page
- Click on the link received after following your appointment
If you are unable to answer the question, a friend or family member is welcome to respond on your behalf.
Your response is anonymous for any of the Friends & Family Test.
How will the results be used?
The information will give your GP practice invaluable feedback on what you think of the care and treatment you have received, which along with existing ways of gathering feedback, will help the surgery to make improvements for patients.
