Pharmacist Team
Meet Our Clinical Pharmacy Team
Our Clinical Pharmacy Team plays an important role in helping our patients get the best from their medicines. Our qualified clinical pharmacists work alongside our GPs and wider practice team to support safe, effective and appropriate prescribing.
Working remotely from the surgery, our pharmacists undertake a range of clinical and medicines management activities, including:
- Structured medication reviews – reviewing your medicines to make sure they remain appropriate for your needs and are working effectively.
- Medicines safety monitoring – checking that medicines are being prescribed and monitored safely.
- Prescribing audits and reviews – helping us maintain high standards of prescribing and improve patient safety.
- Advice and support with medicines – helping to identify potential side effects, interactions or other concerns relating to your medication.
What happens during a medication review?
You may be contacted by one of our clinical pharmacists to arrange a telephone medication review. These appointments may take place during the week or on a Saturday, depending on availability. Your review will usually last between 10 and 30 minutes, depending on the number and complexity of the medicines you take.
During your appointment, the pharmacist will talk through your current medication with you, check that it remains suitable for your needs and give you an opportunity to raise any questions or concerns. This may include discussing side effects, how you take your medicines or whether there are any difficulties with your current treatment.
The aim is to help you use your medicines safely and effectively, while making sure your treatment continues to meet your individual needs. If any changes to your medication are recommended, these will be discussed with you, with further advice or support from your GP where needed.
How does our pharmacy team support the surgery?
By working alongside our GPs, our clinical pharmacists provide specialist expertise in medicines management and help us maintain safe prescribing practices across the practice.
Their work supports the wider clinical team, allowing our GPs to focus their time on patients who need a GP consultation, while ensuring that medicines reviews remain an important part of your ongoing care.
